We strive to help you “Recapture Your Life”. Starting from the initial conversation over the phone, our office staff will guide you through the health care process. Once becoming an established patient our experienced therapists will provide you with the personalized care to return you to the top of your game.
 

As a courtesy to our patients, we contact your health insurance to find out what your physical therapy benefit consists of, what your referral or authorization requirements are, and what your financial responsibility is towards each physical therapy visit. We do encourage you to verify the information with your insurance carrier independently as we are not responsible for any error in benefit or referral information relayed to us by your insurance carrier.

All copays, coinsurances, and deductibles are due at the time of the visit.

What to Expect at Your First Physical Therapy Visit:

​

You will have a 1 hour assessment with a physical therapist to discuss the reason for your visit, perform a comprehensive physical therapy evaluation that includes a movement screen, analysis of your strength, flexibility, and other pertinent tests for your diagnosis. Your physical therapist will then devise a treatment plan and timeline for your rehabilitation.

We encourage you to wear loose fitting clothes with easy access to the area that needs to be evaluated and treated. We do have rooms available to change in.

​

Please bring with you:

​

  • Pertinent tests results and doctors notes (X-rays, MRIs, surgical notes, etc)

  • Physical therapy prescription written by your physician if you were given one

  • Your insurance card and photo ID

  • Intake forms listed below (please note these can also be completed in the office on the day of your first appointment, please arrive 15 minutes early to complete them)

For your convenience, we have supplied these patient forms that are ready to fill out and print to make your visit to the office paperwork free. Please download ALL the forms and fill them out before you arrive.
